Instructions for re-registration for existing members:

1. Please read over the Dues/Payment information on the cover page for the application. Click here for the cover page

2. Please review the rules with your swimmer Click here for the rules

3. Please print and fill out:

   • 2 pages of the application Click here for the application

   • Publicity Form (If not completed in prior years) Click here for the publicity form

   • USA 2009 Athlete Registration form Click here for the USA app

4. Payment should be by check (payable to Falcon Aquatics) in full, or a minimum of $150. Click here for more information

5. Please mail your check and the forms to:
