Falcon Aquatics Swim Club

Instructions for Registering New Swimmers

  1. Please read over the Dues/Payment information on the cover page for the application. This sheet will explain the dues schedule, payment options and payment methods. This can be found here.

  2. Please review the rules with your swimmer. It is important that each swimmer and parents understands these rules. They are enforced.

  3. Please print and fill out:

    • Two pages of the application. Both pages must be completed in order for your swimmer to enter the pool. It is very important that we have this information in case of emergencies, cancellations, etc. Click here for the application

    • Publicity Form. This for gives the club permission to use your child’s name in newspaper articles, the website, etc. We will not publish your child’s name without your permission. Click here for the publicity form

    • USA 2009 Athlete Registration form. This is also very important. We must have this filled out completely for insurance purposes. Click here for the USA app

  1. Payment should be by check (payable to Falcon Aquatics) and either in full, or a minimum of $150. Click here for more detail
